Midwest Transplant Network invites you to participate in its Donor Family Memorial Quilt. This quilt is dedicated to the memory of those who have given gifts of life through organ, tissue and eye donation. Your quilt square will commemorate the life of your loved one. The Quilt will be displayed within the MTN service area (all of Kansas and the western part of Missouri) to help spread the message of organ and tissue donation and to honor and remember those who have given gifts of life.
Size
Your quilt square must be an 8” square, allowing for a half inch seam on all four sides and the design must be within the center 7 inches. This leaves ½ inch on each side so that your square can be stitched onto the Quilt. It is very important that your square meets the size guidelines (see sample pattern) or it will be very difficult to include and may be returned.
Fabric and Sewing- The Quilt travels frequently. Please sew everything securely and avoid using glue, as it may dry out over time. If you are including a photograph on the square, please have it laminated or transferred directly onto the fabric.
- You may use any fabric color or type including sentimental materials such as your loved one’s favorite article of clothing, blanket, uniform, etc.
- You may use crayon, paint, permanent marker, glitter, thread or any other medium to design the square.
- Feel free to include your loved one’s name and dates of birth and death, if you wish.
- Do not finish the square with quilting or backing.
- If you are unsure what to do, ask a friend or relative to help, or look through other donor family quilt squares.
Can I make more than one quilt square?
We are asking families to send in only one square for each donor. You may want to ask other family members for their input when designing your square.
Story
Each family is invited to share a 100 word story to go with your loved one’s quilt square. This can be a story about your loved one, the design of the square or what the project or donation means to you. The story will be displayed online along with a picture of your square. You may send your story along with your square, or you may wait and send it in at a later time.
Release Form
Please print and complete the Quilt Release Form and mail it to MTN with your quilt square. If you are unable to print the form please email us at info@mwtn.org or call 913.262.1668, and we will mail you a copy. This release form grants us written permission to display your loved one’s quilt square in public and to publish your quilt square and story online.
Submitting your quilt square
Please place your completed square in a zippered plastic bag to ensure that it arrives clean and safe. Include the Quilt Release Form and story (if you choose). You may bring them to the next MTN Gift of Life Memorial Service or send by registered mail or insured carrier to:
